Event Description

This educational activity aims to inform the learner about the status of diabetes and chronic kidney disease in primary care. This activity will explore barriers and facilitators to promote patient engagement in treatment. This activity will focus on performance improvement methods and activities which promote health among patients with diabetes.

Objectives

  1. Describe barriers and strategies for clinical care teams in the screening, diagnosis, and treatment of patients with diabetes and chronic kidney disease (CKD).
  2. Utilize new methods to identify how diabetes and CKD disproportionately affect racial and ethnic minorities in clinical workflows.
  3. Explain processes for screening, diagnosis, and raising awareness about diabetes and its connection to chronic kidney disease (CKD).
