Target Audience

This knowledge-based activity is designed to provide nurses, pharmacists, dietitians, physicians, PAs, NPs, and other healthcare professionals with the most current trends and continuum-of-treatment strategies for patients with diabetes.

Event Description

This conference is designed to provide diabetes best practices for healthcare professionals. Participants will gain knowledge about specific topics in the field of diabetes management. It is imperative that providers remain educated in all aspects of their patients’ care, from clinical management to behavior and quality of life. We invite you to attend any breakout session that aligns with your interests and professional goals. While all sessions are open to everyone, the recommendations listed below are based on the intended target audience for each session. These suggestions are designed to help you get the most relevant and valuable experience from the conference. Morning Sessions Troubleshooting Continuous Glucose Monitors; Heather Venoy, RDN, LDN, CDCES Target audience includes LPN, RN, CMA (triage nurses, school nurses, clinic nurses) / providers new to diabetes technology GLP-1 Management; Julie Pruitt, PharmD Target audience includes PharmD, APP's, MD's, CDCES (RD/RN) Afternoon Sessions Words That Move Care Forward: Building Trust Across Visits, Messages, and Systems in Diabetes Care; Carrie Keyes, MMS, PA-C Target audience includes APP's, MD's, CDCES (RD/RN), triage nurses, PharmD Understanding Ketones and DKA Prevention; Courtney Edwards, PA-C Target audience includes LPN, RN, CMA (triage nurses, school nurses, clinic nurses) / RD / PharmD

Objectives

  1. Describe how expanding diabetes technology management in primary care, through CGM and Automated Insulin Delivery (AID), improves care access and outcomes when delivered through a team based model that includes CDCES expertise.
  2. Identify current Automated Insulin Delivery Systems by describing key algorithm features and indications for Type 1 and Type 2 diabetes.
  3. Apply troubleshooting strategies to address common continuous glucose monitor performance and reliability issues.
  4. Apply evidence-based strategies for selecting, initiating, and managing GLP-1 therapies in patients with diabetes.
  5. Apply a structured framework to respond effectively to emotionally charged patient messages.
  6. Identify risk factors, warning signs, and prevention strategies for diabetic ketoacidosis.
  7. Describe the stages of Type 1 diabetes and the clinical benefits of early detection.
  8. Differentiate diabetes management fatigue from nonadherence and apply strategies to support affected individuals.
