Event Description: This program is for NC Health Department employees only. Beginning in 2026, HRRN transitions from pay for reporting to pay for performance, reinforcing NC Medicaid’s commitment to addressing social drivers of health. This session examines required screening components, best practices for documentation, and effective approaches for follow up and linkage to community based resources.
There is no charge for this program. Course registration is generously supported by the North Carolina Alliance of Public Health Agencies.
Objectives
Explain the HRRN measure definition and the new gap to goal scoring structure.
Implement standardized screening processes that meet Medicaid specifications.
Use workflows to ensure follow up attempts and successful completion of screenings.
Strengthen partnerships with community agencies to close the loop on referrals.
