Event Description:  PLEASE READ THESE INSTRUCTIONS BEFORE MOVING FORWARD TO DATE SELECTION

HPBOC Tumor Boards are held on Tuesdays from 12:00 – 1:00 on the 2nd floor of the Atrium Health Wake Forest Baptist Comprehensive Cancer Center in conference rooms 2A/2B. Exhibits are held in Conference Rooms 2C & 2D.

NOTE: During the registration process, PLEASE INCLUDE YOUR ORGANIZATION - THIS IS ESSENTIAL FOR US TO BE ABLE TO MATCH PAYMENTS TO THE CORRECT VENDOR AND DATES. Once you have made your selections, you may immediately pay by credit card but ONLY during the registration process. You cannot leave the registration page and then re-enter to pay.

Please download and complete the exhibit form under “download brochure” on the left side of the registration page. This is a fillable PDF form. You may have to download the form for it to appear fillable. You will also find our institutional W9 beneath the exhibit form. Once the exhibit form has been completed, you will need to scan and email that form to Gail Pawlik at gpawlik@wakehealth.edu. Please include your company's name on this form.

NOTE: DUE TO THE LIMITED NUMBER OF AVAILABLE DATES, YOU MAY ONLY SELECT TWO DATES. EXHIBIT PAYMENT MUST BE RECEIVED ONE MONTH PRIOR TO THE HELD DATE OTHERWISE THE DATE WILL BE RELEASED BACK INTO THE OPEN EXHIBIT POOL.

If you need to pay via credit card after leaving the registration page, please contact Gail Pawlik 336-713-7761. She will take your credit card information over the phone. Once you have selected your date, you will receive a letter confirming your date selection.
